Fails to observe properly / controlled / uncontrolled intersections

When approaching any intersection, including a railway crossing, always look Left-Straight-Right-Left before crossing. The reason for this is that red lights and stop signs do not stop everyone. Also, sometimes railway warning lights can malfunction or are not even present. We look left first as that is the first direction that a vehicle would hit us from. We then look straight ahead to see anyone turning in front of us and to ensure we continue to maintain our lane position. We look to the right next as that is the last path of travel that we cross. Finally, we do a double check to the left because of the delay it takes for us to do all this. Failing to observe properly consistently could be a cause for failure.
